Dear Mr. Leal:

Please find, enclose, the application form for a Canadian passport which you had provided for me to complete in order for me to obtain a travel document from the Canadian Consulate.

I have completed the application to the best of my ability, providing only true and correct information. However, please note the following points:

1. I cannot provide a signature in section 1 because I am not a Canadian citizen.
2. Section 1 also asks for my surname at birth and my former surname. I decline to provide this information on the grounds that I may incriminate myself. As I have stated in the past: in 1996 I changed my name. I am not required under any law to provide my name prior to that time.
3. I cannot provide proof of Canadian citizenship as required for section 4 because I am not a Canadian citizen.
4. I currently have no documents to support identity as required by section 5.
5. I have been in custody for 3½ years, and so, have not worked in the last 2 years.
6. I do not know with certainty my mother's maiden name. I do not know my mother.
7. I have not references who have known me for at least two years and who can state with any certainty any information about me.
